How on earth do you keep the pinion from falling off?

Supposedly. Doesn't seem to work. Still falls off after a few minutes
use

Usually there is a flat on the shaft that the grub screw is supposed to
sit on. If you are finding that and it is still getting loose then put a
touch of threadlock (Locktite blue stuff NOT red or you'll never get it
off) on the thread of the grub screw before you put it back into the
pinion, then refit doing it up tight on the flat.

Make sure you got a STEEL grubscrew and wrench in GOOD condition.
That way you can thighten it very hrd on the flat part of the shaft.

Try using some loctite (thread locking glue) on yer nut/threaded shaft.
