Comes with an order form Envelope, Warranty card, Extra Prop, Mounting Hardware.... It looks like everything just as it came from the factory.
Also included is the 8572 Throttle linkage Kit which is still unopened in the package.
The serial number as stemped on the mount is 0 6128
As you can see, the middle section of the foam inserts has started to deteriorate. I dusted the foam "powder" off of this one before I took the photos, and I will wrap it in plastic before I put it back in the box to ship it.
