I have been tasked to produce a questionnaire for our Flying Club. The purpose is to gather data and feedback from the members and to get ideas from the members for ways in which we can improve the club, the flying site, services which the club offers etc etc.
Before I start with a blank sheet and my own ideas I thought I would ask if anyone already has a good example questionnaire they have used, which worked well, and which they can provide to me.
Also, any good pointers for any specific topics for the questionnaire and/or successful ways to best use the questionnaire, all would be much appreciated.

some ideas we used (sorry dont have the questionnaire anymore)
#1 length of grass
#2 pit lines cut in grass (ie pit line is let to grow to longer length making it easier to distinguish)
#3 leveling of the field
#4 removal of obstructions...trees, bushes, etc
#5 instructor bulletin board with pegs to show which instructors are currently at the field (also for posting of necessary information)
#6 designated flying postions (we put 2ft x 2ft concrete slabs down to limit the # of airplanes allowed to be flying at once
#7 maximum # of aircraft that should be allowed in the air at one time
#8 flying pattern, clockwise or counterclockwise
#9 specified times for 3D/aerobatic flying as it always interrupts the normal flying pattern
#10 types of mufflers/pipes to be used (based mainly on your city/country's dB codes)
#11 Non-landing gear aircraft (at my flying site, glow/gas/diesel powered aircraft are required to have LG, electrics being the exception to the rule)
#12 covering for the ground in the pit area to protect against killing the grass and causing soil erosion/pitting/etc
#13 specified days for training new pilots
#14 acquisition of buddy boxes/trainer cords out of club funds to train new pilots and get more people interested
#15 frequency stand position, clearly marked etc
#16 sign placement (we put up "Pilots only beyond this point" signs to give added safety to the spectators...also positon of flying field rules board
